As it turns out, it was an infection that had solidified. It just kept getting bigger as more puss developed around it.
It was about the sized of a garbonzo bean and looked like a little brain.
I have him isolated from his mate and I have to clean and pack the area with antibiotic ointment once a day.
As long as I can get him to start eating again, the vet thinks he should recover quite well.
He is getting better by the day -

Last night it took awhile, but I was able to get him eat a couple of wax worms.
I put a few in his tank this morning before I left for work.
He is kinda of listless, but I think he'll come around.
I'll keep you all posted
Now if he will fatten up everything will be fine.
Of course now the stray cat we feed is sick, so looks like we are headed back to the vets. Just another day in the life of a person with too many animals.
